Eye complications are frequent in diabetes and can lead to partial or even total loss of vision. However, much can be done to prevent the worst outcome. Cataract is twice as common in diabetic people as in people without diabetes but can be effectively cured by surgery. The severest complication, however, is diabetic retinopathy. This is a progressive disease involving damage to the retina, the delicate light‑sensitive part at the back of the eye which picks up the images of people and objects around us, like the film in a camera. Adequate treatment by laser photocoagulation can lower the risk of blindness, but often diabetic retinopathy does not manifest itself with loss of vision until it is almost too late to intervene. In other words, if you have diabetes you may have sight­threatening retinopathy and be unaware of it. The only way to reduce the risk of loss of vision due to diabetic retinopathy is to have your eyes checked regularly by the doctor who looks after your diabetes or by the specialist to whom you will be referred. 

A yearly examination is usually sufficient to reduce the chances of loss of vision, as diabetic retinopathy has a relatively slow progression. However, in some cases, more frequent visits may be necessary, for example during pregnancy or prolonged illness. 

Before having your eyes examined, you should inform the specialist of whether you or anybody in your family suffers from high blood pressure or glaucoma (that is high pressure inside your eye leading to damage of the nerves). You should also state any recent changes in or problems with your vision. 

Examination to prevent diabetes‑related blindness should include: testing of visual acuity (reading eye chart), dilatation of your pupils with eye drops and examination of the back of your eyes with appropriate instruments (ophthalmoscopes). Alternatively, photographs can be taken of your retinas for later assessment by specialists. Eye photography and examination does not damage the eye. Pupil dilatation will last for a few hours and will interfere with your ability to read and drive.

Although your doctor will make sure that your eyes are checked regularly and thoroughly, you can help him/her by keeping your appointments and promptly reporting any change in vision. Please tell your doctor if you have encountered any problems when having your eyes examined or if you feel that you did not receive sufficient attention. Your cooperation in improving existing facilities will be much appreciated.
